Insurance Claim Appeal Process

Overview

The following are the appeal procedures to follow when dissatisfied with a claim determination.

Initial Review of Claim Determination

A plan participant who believes an error has been made in the benefit amount allowed or disallowed must contact the appropriate managed care plan or plan administrator within 180 days of the date of the initial claim determination.

Use of the Benefit Estimator does not replace a SURS counseling appointment.

The Benefit Estimator is a helpful tool that can be used by members when they do not meet the criteria for a counseling appointment. Please keep in mind that the online estimator cannot accurately calculate estimates for members who have part-time service, reciprocal service or police/fire service.
